It might have been.

Here's what I've discovered since I originally posted my outline numbering 
problem.

1. Outline numbering is set up, but is turned off for some of the headings. It 
doesn't look like it's been turned off intentionally because there's no 
apparent rhyme or reason to why some headings are turned on and others aren't.

2. In Linux, I can select the whole document and turn numbering on, and the 
numbers appear as expected.

3. In Windows, I can't select multiple headings. I have to go through and turn 
numbering on one heading at a time.
