Just to let you know, that actually wasn't the problem. ;) That was
how I created them. The headers weren't appearing in the contents. I
had to add a character to the line that contained the header to make
it read (in this case, a space.)

Problem probably is resolved, when you use Tools|Outline numbering to
link numbering to the headers.
