Here's what I get breaking down your country lists by continent:

So the comma is certainly predominant in Europe, but I had no idea the usage was so variable. Fascinating. Thanks for the clue!

It would be interesting to break it down by colonial influences, but I don't remember enough history to do it off the top of my head. Now I'll have to go see if I can learn where the two notations came from.
