I don't know if it's at all relevant, but as the last couple of digests 
seem to have dropped this thread, I'll say it. Which is that a few 
times I have had problems with the occasional graphic (not, I admit, 
the whole document) not appearing on printout. I eventually traced it 
to a printer driver issue, Epson in that case.

Can I suggest you try installing a postscript printer driver, set to 
print to file, and see if the postscript generated is correct. (I use 
the Apple colour laserwriter 12/600 driver) This might provide a 
pointer as to where the fault lies, and if it works may provide a 
possible workaround, albeit a bit inconvenient.
