I also experienced this frustrating problem.  Even when you use Styles and
Formatting (Format) and set the First Page, followed by Default pages and
then input the headers on each page.  You can save it and it is OK but once
you close it and open it again it has not held your save.  And the default
page headers disappear and the first page header appears on every page!

I ran across the same issue, maybe I can shed some light.

I have a customer where I'm migrating them to Open Office 2.1 from Word
2000.  They have many documents that were created using Word.  as part of
the migration we were opening/editing the documents using both programs.

What we observed is that when we open a MS document with OO 2.1 everything
looks good, however when we close/save the document and then reopen the
document with MS Word the header info is gone.  The same situation occurs
when the document is edited with headers created using OO and opened with MS
Word.
