Have your tried using a frame, putting in the title (header) in the frame
and then inserting a Table with two columns?

styles, but often it feels like if I could do it with paragraph styles,

Next idea was frames, but that also failed. Yes, I can have two (for
example) columns in a frame, BUT I can not mix two-columns text with
one-column text in it.
