Find the Controls icon in this toolbar and use the arrow next to this to show a drop-down menu of form controls. You can alternatively drag this menu off as a separate Form Controls toolbar (and then hide the Insert toolbar?). This menu or toolbar contains things such as check boxes, text boxes, option buttons, list boxes, and so on, to help you. Look up "form controls" in the help for more details. If you use File | New > (or the drop-down menu from the arrow next to the New button) and select XML Form Document, the Form Controls toolbar will open automatically. I trust this helps.
