Your attachment did not make it through the list server, in fact, so
no: we cannot see what you are trying to do.
If you use Form Controls in a Writer text document, all parts of the
resulting document will be available for your users to modify when
they complete it - not just the areas you specify. This is probably
not what you require.
In Writer, choose File | New > | XML Form Document (or use the
drop-down menu next to the New button). Use Form Controls here to
create what you want, possibly with text boxes. If you save this as
a Writer document, your users will be able to complete it and print
it (or even e-mail it back) if they have OpenOffice installed.
You can export this document as PDF, and Adobe Reader users will be
able to complete it and to print the completed version. They would
need Adobe Acrobat (not freeware) (or similar) of they were to need
to edit your PDF file itself.
