This may be relevant here:  I'm working on a large project converting InfoPath
forms to OOo's XForms.  I've noticed the nearly complete lack of documentation
on this, so I've been taking notes on things as I go along at
http://compiler-error.blogspot.com .  I've also started writing a "how-to"
that's somewhat specific to my project, but also fairly general at
http://openmrs.org/wiki/Creating_Forms_in_OpenOffice .

If you're interesting in using XForms (and they certainly seem capable of doing
what the OP is trying to do with some work), perhaps those links will help some.
 I hope to make more coherent XForms documentation in the future.
