Hi Michael

As no one else has responded, look in Help under "Labels" The facility
to do this is built into OOo

You may well be able to find a template for that label. If it is not
included, have a look at
http://www.worldlabel.com/Pages/openoffice-template.htm

Come back to the list if you have problems actually doing it.
