The document has a TOC and chapter headers that are in a different
paragraph
format (of course) from everything else.  Could that be part of the
problem?

there is a known problem selecting all text when the very first line is a table of contents (TOC), see http://www.openoffice.org/issues/show_bug.cgi?id=17949
Workaround: never insert a TOC at the very beginning of a document.

To reset your TOC from read-only to allow editing, right-click the TOC, choose Edit Index/Table, then uncheck the Protected against manual changes box.
