there is a known problem selecting all text when the very first line is
a table of contents (TOC), see

Workaround: never insert a TOC at the very beginning of a document.
To reset your TOC from read-only to allow editing, right-click the TOC,
choose Edit Index/Table, then uncheck the Protected against manual
changes box.
