This should be straightforward: I'm not sure which part is puzzling you.
o Open a text document. (This is a dummy: you can discard it later.)
o Go to Insert | Envelope... and choose the required parameters.
o Select New Doc. (not Insert).
o Fill in your return address.
o Either paste your graphic from the clipboard or use Insert | Picture
> | From File... to insert the graphic.
o Right-click the graphic and select a suitable option for Anchor (To
Page?).
o Resize the graphic and drag it into position.
o Go to File | Templates > | Save..., give the template a name, and
save it in My Templates.
o Discard the dummy document.
To use the template, go to File | New > | Templates and Documents and
double-click the saved template. Edit the new envelope and save or
print it as required.
