That's done by adding a frame around the picture and the caption is text you enter in that frame. The caption category is the pre-defined text that appears at the beginning of the caption. It is a word like "Illustration", "Drawing", "Table" or "Text". As explained earlier, you can use other words or you can choose not to use a category.
