Probably it's just habit. I think most people responding are concentrating on the body of the post and when the response is finished just hit the reply button. I try to remember to check the subject line again before posting a response, but sometimes it slips past.

Also, with interspersed or bottom posted replies, the responder is working away from the subject line and some responders are strongly averse to changing subject lines or "hijacking" a thread.

A couple weeks isn't much time to overcome old habits; I've never seen another list with so many missing subjects as this one.
