There's a regression bug in OOo 2.4 with regards to headers and footers.
When the document contains a Table of Contents, then any pages after
that do not have headers and footers in MS-Word. It used to work fine in
OOo 2.0

Ie confirmed your issue and noticed that the same problem exists with
save as .doc in 3.0 Beta.
