it took some time, but thanks to your details debug output I was able to trace the problem and fix the bug. As I was expecting, in was in the to/from parser.
Please update from CVS and give it a try.

uac is called like:

The INVITE was most likely produced by a "Aastra Intelligate" SIP UA since the particular customer has one of these registered right now.
