in the latest devel version, the Radius User-Name AVP is not included in
the default ACC set. See:
http://www.openser.org/docs/modules/1.2.x/acc.html#AEN54
so, you can use extra acc for Radius to push whatever value you need.

reading the source of auth_radius module i think this is not possible
right now.
the User-Name is filled by the digest credential username + realm.
could be usefull to have the possibility of customize this.
