I'd suggest you look closer at the call setup process to see where exactly it makes sense to generate accounting records. If there really isn't an opportunity to due what you want during call setup perhaps you should consider call accounting in both SER and Asterisk.

    Accountig start after 200 OK and you couldn't use DTMF before it.

    in this way you should to ask Asterisk team, how to use accounting
    there.
