I have noticed after full power failiure (all nodes simultaneously - labs don't 
have UPS yet) one of my VMs was in that state, and I had to restart VDSM in 
order the engine to notice the crash.

Definitely , there is something phishy here...

I guess Sahina & Sandro can take a look in your case..
