I've implemented code which splits PDFs based on bookmarks, combines PDFs, and add bookmarks. In my experience, PDFBox was able to do everything with bookmarks which is specified in the PDF Specification (ISO32000) and I've contributed code in the past which maps bookmarks to page numbers for quick and easy reference. I think the one word answer to your question is "yes" and Jeremias and Maruan both went into specifics on how to get started.
Check out the examples (in org.apache.pdfbox.examples if I recall correctly) and you should be able to hack those into a program that will meet your needs. Let us know if you need any clarification on anything.
