I'm not an annotation expert, but according to the spec both the Rect and the QuadPoints values are specified in default user space which doesn't include any rotation or scaling. But I have no clue where to put these information instead. Can you create a sample pdf with such an annotation using acrobat or something similar so that we can have a look how it looks like?
