I am not saying that PDPattern should have a /Length. PDTilingPattern
should and in version pdfbox-app-2.0.4-20161109.083729-125.jar
PDTilingPattern dose not put a /Length, which is one of my problems
every thing else is ok except that /Length is missing from PDTilingPattern.
Unfortunately the mailing list will not let me put attachments, so I can
not show you a screen capture of the result from PDFDebugger, which
would make it very clear what I am talking about.
Matrix is just a hint, it would be a nice to have. :)

Don't confuse the pattern colorspace with the TilingPattern. The
second one has a /Length value.
Also according to the same document, all but 2 of the options are
compulsory "Type" being one and the other is "Matrix" (which is missing)
Matrix is not compulsory.
I might add I am also looking at gs-bugzilla691715-1.pdf to spot what
is wrong and then looking stuff up, too see if it is needed.

pdfbox-app-2.0.1.jar
I really suggest you run my example, and show the result file in
PDFDebugger. My example is less complex. And even if this may not be
obvious, I made it for you :-)
