Hello Constantine, which version of PDFBox are you using?
Flatten of AcroForm works from the Fields entry so if there are no entries and only Widget Annotations they remain as is. We introduced a number of fixups last year (PDFBox 2.0.22) which can be exectuted and build the Fields entry from existing widget annotations.
