This is extremely difficult. You'd probably need years to write this from scratch unless it's files that are all similar. Use a commercial product.
To understand why, 1) read stackoverflow answers that show how to add tags from scratch, 2) use PDFDebugger to look at a file with tags, look at the content stream and search for MCID, then look at the structure tree (you need to change the "view" in the menu).
