OK thank very much for the help
so far.
I've installed openser
and enabled the nathelper module
and I know that openser can communicate
with rtpproxy through the Unix socket.

However am still not getting any audio
2 things:
1) am not sure I understand "Basically, you need to indicate direction the 
message is going when calling force_rtp_proxy()"
2) I have a feeling that I should define the port
range over that the VoIP handsets communicate (2048:3329),
however I have no idea where I am supposed to specify that.

Most likely is that you are not using the gateway mode correctly.
Basically, you need to indicate direction the message is going when calling 
force_rtp_proxy(). Please see the modules/nathelper/examples/alg.cfg file for 
details.
