The problem was related to Thunderbird - Lightning cache. Solved deleting %APPDATA%\Thunderbird\Profiles\<PROFILE>\calendar-data\cache.sqlite

If i watch the same appointment via web interfaces, the reminder is correctly set. Tried to watch the logs and the record on PostgreSql and all look right. On my iphone 4 with ios 5.0.1 have the same issue.
