Apple's idiosyncratic Perl installation has historically been intolerant of user installation of additional modules or updates of existing modules. Maybe High Sierra breaks with that tradition, but I doubt it.

For people who do not want to pay the small price for Server, MacPorts or Homebrew are great alternatives for building a distinct environment of open source software (including, if you want, a current and less pathologically configured Perl environment) and can install SpamAssassin functionally.
