Note that test.surbl.org won't work because surbl.org is not blacklisted, and the code reduces a .com to the second level before checking. Instead use: http://surbl-org-permanent-test-point-MUNGED.com/ or: http://127.0.0.2-MUNGED/ without the "-MUNGED"s.
