well, here it looks like this:

MUA -> senders MTA -> my external MTA -(fetchmail)-> my internal MTA 
-> one internal hop through spamassassin -> one internal hop through 
antivirus -> my MUA

and at my workplace its a similar setup, without the fetchmail.
