You'll see that if you happen to try a restart while a message is being processed and the scan doesn't complete within the time the init script is willing to wait. That reports FAILED because the process is still there. Then the restart doesn't start another copy, and sees the old one, and reports OK. Then the running SA exits when the scan of the current message finishes.
