When I've hacked about with the rules and introduced something malformed I sometimes got MISSING_SUBJECT and its ilk firing. Don't know if you may be seeing something similar - try linting your rules and see if there's any typos.

I ran Lint, and I had a few config errors (badly written whitelist_from_rcvd lines) which I've fixed. Lint did catch the non_empty_body meta rule that is part of the NOT clause for Empty_Message
