I've started getting spam sneaking through SpamAssassin with no
tagging.  When I look in the logs, I can see that it gets scored, but
the headers I've asked SA to add are simply not added.

I've got the following in my local.cf:

clear_headers
add_header spam Flag _YESNOCAPS_
add_header all Level _STARS(*)_
add_header all Status _YESNO_, bayes=_BAYES_ score=_SCORE_ required=_REQD_ 
tests=_TESTSSCORES(,)_ autolearn=_AUTOLERRN_ version=_VERSION_
rewrite_header Subject [SPAM:_STARS(*)_]

What's going on here that prevents the tagging from happening?  Thanks!
