than you likely know the reason

SpamAssassin needs to know what hosts are your "trusted_network"
and " internal_networks" to know which is the LASTEXT to fire RBL
requests against

This is your problem, Spamassassin simply cannot handle this. It's
documenting an SMTP transaction from outlook, but recording the IP and
reverse dns of one of your own servers.  You don't appear to have any
information about the last external IP address at all.

I agree, i also suspect this is the problem. I will stop the ipv6 converter and will let you know if the PBL rule works as expected again.
