My other experiences with environment variables and non-env variables with Apache was that you can set a variable to nothing but it is still considered "set" (or "defined", if you prefer). In this case, it is not. I tried your incantation above, and it works. It does everything I want except to return a 403 error, which I can probably add with [F] in addition. In my case, I want other sections of the conf file to allow arbitrary 403 handling, and I don't want to create RewriteRule /403.html - [L] rules for all of them. So, I'll live with the 200 response.
