Btw, this might happen when you copy text from, say, a web page. The page may look normal, but contain non-standard versions of common characters such as double quotes/apostrophes/three consecutive dots/other. They may look ok in your editor, but they'll have non-standard char codes and could lead to some non-obvious strangeness (such as what you described).
