I opened JIRA issue 3181: https://issues.apache.org/jira/browse/WICKET-3181. I attached a ZIP file with complete source to reproduce the issue and instructions on how to run it (I am sure you didn't need them).
Please let me know if you can think of any other solutions for displaying a modal window from nested upload form.
