One solution is to write an event listener component (for save event for instance) that checks if an attachment is uploaded and scans it using your antivirus API. Unfortunately the documentation is a bit outdated http://extensions.xwiki.org/xwiki/bin/view/Extension/Observation+Module+Local .
Maybe others have better suggestions.
