The error in reading prescriptions such as the one that killed the child might have been avoided if the ANMC Metric Editorial Guide of August 1990 was followed.  The Guide clearly states in Section 3.5:
 
"A zero before the decimal point should be used in numbers between 1 and -1 to prevent the possibility that a faint decimal point will be overlooked."
 
This should be used no matter which units or system of units are used.  This issue is completely separate from the mbar vs hPa vs kPa issue.
