root is not blocked for this particular system.  Like I said, after 
'vagrant up' fails, I can do 'vagrant ssh' successfully.  This allows for 
me to login as the root user no problem.  I can also login with a normal 
ssh command as the root user, no problem.

Something must be up with the code that does the initial provisioning with 
SSH.  Where in the code base does it do the check for the prompt?  I assume 
that there is a regex or some other pattern match somewhere.
