Personally I don't like things to be moderated, I'd never contribute to a
thing like that. Things like stackoverflow, wikipedia, gnome wiki & co work
because there's no moderation.
At last one can have a login account to reduce spam (also openid if
possible), and a report flag. Other than that, the best thing would to have
_similar_ mechanisms of  stackoverflow applied to every page of the docs:
- Downvoted comments are hidden
- People can earn badges (like Novice, Guru, etc.) to attract people to
comment more
- Comments to comments, so that main comments are relative to documenting
stuff and not only replying to people.

I'm also in favor of very simple stuff like <code>....</code> and that's it.
