I thought Varnish 4.0.1 has support for IMS.  But looks like varnish is not 
sending the IMS header to the backend.  Or I'm missing something.

Is IMS enabled in Varnish ? or is it still in the experimental branch.

Given varnish is being used to serve images, we'd like to cut down on the 
traffic between varnish cache and backend server as much as possible.
