I've been doing something similar, and am running into the same problem.

Presumably, the solution is to have the "Include toolbar button to
show/hide" option checked, since that is what normally makes it an
optional window. (It sounds like you had done this.)

The actual problem I'm finding is that when you save the module, if a
Map Window is an element of of Toolbar Menu, that option automatically
becomes unchecked.

So: we have a bug. I can provide a copy of my module to show off the
behavior.

Other comment on the Toolbar Menu: It would be nice if the Button Text
would show up in the main edit window, so that you can identify
multiple ones.
