D: is probably your virtual CD-ROM drive, with the
VBoxGuestAdditions.iso mounted in it. The drive icon should be
different than that of a harddisk.

After installation of the guest additions, it is not needed
anymore. 

To remove, you can right-click on the drive in Windows Explorer
and choose eject. Then use the VM menu: Devices / CD/DVD drives
and remove the check mark next to VBoxGuestAdditions.iso
