How do I attach it using the GUI or vboxmanage?

I already stated that the drive does not show up in the GUI.

As for vboxmanage I have no clue.  I tried:

$ vboxmanage modifyvm -dvd host:hdb OpenBSD_40-T4
$ vboxmanage modifyvm -dvd host:/dev/hdb OpenBSD_40-T4
$ vboxmanage modifyvm -dvd host:/dev/cdrom OpenBSD_40-T4
