I haven't seen your previous posts, so please forgive me if I ask something
you've already specified.

Not to sound like a broken record, but...

...are you running Redhat Linux? Installed qmail from RPMs? Then maybe a
cron job is breaking your /var/qmail/users/assign file on a periodic basis.

Only if the virtualdomain doesn't have an A record that points to the mail
handling machine. First it checks for MX; if not found, it checks for A.
