It should have all authentication modules installed should it not (And which
ones are loaded controlled by your imapd.conf file)? I will look into this
when I get home tonight.

Well, have you recompiled your courier IMAP after changing your
authentication format to mysql? I think courier is still coded to
authenticate with your cdb files.
