other things to look for:
excessive collisions on the ethernet (unlikely)
Denial of Service attack in progress, look for lots of connections to the server that are doing "nothing"
what is the IP address/name of the MX in question?
what are your concurrencylocal and concurrencyremote settings in /var/qmail/control?
do you have any values set in timeoutremote and timeoutsmtpd?
