Any guess on areas that might be culprits?  Does chkuser have a default
"permit" behavior if it cannot reach IMAP or Sql?  Not sure if there's a

First of all, you should install latest version of chkuser.
Anyway, unique problem I had until now is relative to usage of blank in front or after name, because MySQL strips away leading/following spaces, so a "fakeuser" is found exiting (vpopmail calls returns existing), while all post processing will not found "fakeuser " user.

Vpopmail sql statement should be changed in order to avoid this problem.
