VCL-518 function: Allow global or per-image specification to cause a user
making a normal image reservation to respond to a click-through agreement, and
log agreement in the clickThroughs table.
These files are based on VCL 2.2.1. Most changes in requests.php, also
added a new state to states.php and configuration variables in
conf-default.php.
