Nursing reimbursement in our home infusion enviroment is that the first visit, up to 2 hours, is one fee. After that, each additional hour is an additional fee. We apply for preauthorization for the anticipated length of visit if we know that the visit is going to be longer than 2 hours, such as IVIG or Remicade. In some cases, we need to send the insurer a copy of the nurses notes.

I'm curious how those of you in home health are billing insurance companies for extended duration infusion visits. We are generally paid by the visit, but if in the home for up to 6 hours for an infusion this is not cost effective. I would be interested in learning if others are billing hourly, by the visit, or maybe a combination of the two.
